Will the referee accept his error for continuing on the game. Windsor sustained 2 injuries in that last 15 minutes. Who is accountable?

We can just trust that both teams have done the right thing and if there is an oversight or mistake we can fix it. The mistake was realised in the 75th minute by the referee who made the player go off and wouldn't let Windsor make a change and had to play the last 15 minutes with 10 players.
